I, grant permission to MiXED Magazine. to use three (3) photographs of myself taken by me for the perpetual and exclusive use on Mixed-Magazine.com and its associated web sites including but not limited to any advertising, promotional or publicity purposes.

I hereby waive any right to inspect or approve the aforementioned use of the photograph now or in the future via electronic media, the Internet and any and all other mediums existing today or in the future. I hereby agree to release and hold harmless Mixed Magazine,

from and against any claims, damages or liability arising from or related to the use of the photographs, including but not limited to any distortion, blurring, alteration, optical illusion or use in composite form, either intentionally or otherwise, that may occur or be produced in production of the finished product.

I am 18 years of age and I am competent to contract in my own name. I have read this release before signing below, and I fully understand the contents, meaning and impact of this release. I understand that I am free to address any specific questions regarding this release by submitting those questions in writing prior to signing,and I agree that my failure to do so will be interpreted as a free and knowledgeable acceptance of the terms of this release. I hereby affirm that I am over the age of majority and have the right to contract in my own name. If phishing, hacking and/or fraudulent actions occur with voting, I will be immediately disqualified.

I have read the above authorization, release and agreement, prior to its execution; I fully understand the contents thereof. I am a legal US resident and a natural born female. This agreement shall be binding upon me and my heirs, legal representatives and assigns. I understand that all judging for the contest by Mixed Magazine is final. 50% of contest is based upon public votes, and the judges decision accounts for the other 50%.
